Had a similar situation a couple years back during a remodel. Did all the usual checks—vents, grout, fans—but still had that damp swampy smell lingering around. Turns out moisture had seeped behind the tile through tiny cracks in the caulking around the tub. Ended up stripping it out completely, drying thoroughly with fans and dehumidifiers, and then re-caulking with a high-quality silicone sealant. Smell disappeared after that. Sometimes moisture hides in sneaky spots you'd never think to check...
