What do you document before covering a soft spot in roof sheathing?

A soft or discolored patch in roof sheathing doesn’t, by itself, explain how far the damage goes or what caused it. When a contractor finds one during shingle replacement, what evidence helps distinguish a localized repair from damage that extends beyond the visible patch?

I’d be interested in how the area looks from above and from the attic, and whether its location near an eave, valley, or roof penetration changes what you look for. What photos or measurements should a homeowner ask for before new underlayment and shingles cover the deck, so the repair scope and any limits to the inspection are clear?

For the boundary, I’d ask for a roof sketch with the measured repair area marked, plus photos showing the patch in context and close-ups from above and below where attic access allows. If the underside can’t be seen, that limitation should be noted; a clean-looking top surface alone doesn’t establish that the damage is localized.
