When interior paint blisters or peels, what clues help tell a surface-preparation problem from moisture behind the finish? I’d want to know whether it’s concentrated near a window, on an exterior wall, or in a bathroom, and whether the paint is dry and flaky or feels soft. But I’m not sure how much those details can narrow it down on their own.
What would you inspect or check before scraping and repainting, so a recurring problem doesn’t just get covered up again?

The location and texture clues are useful, but I wouldn’t treat them as enough to separate moisture from poor adhesion. A soft patch or peeling near a window tells you where to look, not whether the wall is still damp or what caused it. Before repainting, the key is to establish whether there’s active moisture behind the finish; otherwise, the same spot may peel again.
